This training discusses the important world of documentation. Documentation is a crucial part of human and social services work. If a professional does not document, it did not happen! This training assists staff with polishing their current documentation skills and learning what is important to include in documentation. This training provides documentation examples, role-plays and nuances of documenting in-person versus phone interactions with clients.

What You'll Earn

Social workers completing this training will receive 3 clinical continuing education clock hours under ACE.

This training is approved for 3 CE clock hours in ethics for counselors, social workers, and marriage and family therapists, as well as substance use counselors in domain TR1 - treatment field related (renewals only) in the state of Ohio.

Jorie Schwartz, LISW-S

Professor, Social Work and Human Services Program, Columbus State Community College

Jorie Schwartz, LISW-S, is an experienced social worker with over 27 years of service in Columbus, Ohio, specializing in community mental health and school-based prevention. An MSW graduate of The Ohio State University, Schwartz has spent the last 12 years shaping the next generation of social workers as a professor in the Social Work and Human Services program at Columbus State Community College. She also serves as a community lecturer at The Ohio State University College of Social Work. Her collaborative and student-centered teaching approach helps foster both academic and personal growth. Schwartz says that her greatest joy comes from those “lightbulb” moments, when students discover their passion for the field. Schwartz is deeply committed to preparing students for impactful careers in social work, guiding them not only through coursework but also in navigating the real-world challenges of the profession. 

Angela Fry, MSW, LISW-S

Professor, Social Work and Human Services Program, Columbus State Community College

Angela Fry, MSW, LISW-S, earned both her BSSW and MSW degrees from The Ohio State University College of Social Work. With a career rooted in trauma and abuse work, Fry has gained extensive experience through her service in various agencies in Columbus, Ohio. Fry began her career at CHOICES, a domestic violence agency, where she worked as a case manager supporting women residing in the shelter. She then spent a decade at Catholic Social Services, taking on diverse roles such as a home-based clinician for families involved with child protective services, individual and couples counselor, group therapy facilitator, and clinical supervisor. Venturing out, Fry worked with children who had been sexually abused and served in foster care. Returning to CHOICES, she served as clinical supervisor for several years and is now a professor in the Social Work and Human Services program at Columbus State Community College. 

Continuing Education Approvals 

The Ohio State University College of Social Work provides approvals for continuing education for counselors, social workers, and marriage and family therapists that are accepted by the state of Ohio Counselor, Social Work, and Marriage and Family Therapist (CSWMFT) Board.

The Ohio State University College of Social Work has been approved by the Ohio Chemical Dependency Professionals (CDP) Board as a provider of Continuing Education, approval number: 50-24068.

The Ohio State University College of Social Work, (Provider: 1628), is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved as ACE providers. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. The Ohio State University College of Social Work maintains responsibility for this course. ACE provider approval period: 4/30/25-4/30/28. Social workers completing this course will receive 3 clinical continuing education credits.

Certificates & CE Reporting 

CE certificates will be available for download in your learner portal within 1 business day after submitting the evaluation survey.

Hours for those with Ohio licenses that are submitted with the evaluation survey will be reported to CE Broker every 14 days (twice per month). 

⚠️ Note: You may only complete this training once for CE credit. You may revisit the content, but only one certificate will be issued and credit will only be reported to CE Broker one time.
